If you do not know which form is appropriate for you, just contact me to ask. Often, I fill out the pertinent form myself while asking the appropriate questions when we meet at our initial consultation, so it is not necessary that you come to the appointment with form already prepared.
However, if you live far outside the Tulsa area or otherwise wish to retain me without meeting in person, you may complete the forms related to your type of case and forward that to me, along with payment, by a variety of means. Methods of payment: Cash (in-person only), money order, check, or by Visa or MasterCard.
